BIRTH AND DEATH RECORDS

All original certificates for anyone that is born or dies in Berlin are filed in the vault. (Even if the person is not a Berlin resident.) Certified copies are available for $10.00 each.

Copies can be requested to be mailed. Send written request to: Town of Berlin

Include in the request for a birth certificate:

                                    the child’s name,

date of birth,

an address to mail the certificate to, and

a check or money order for $10/certificate requested.

Include in the request for a death certificate:

                                    The deceased full name,

                                    date of death,

an address to mail the certificate to, and

a check or money order for $10/certificate requested.
