BIRTH AND DEATH RECORDS

All original certificates for anyone that is born or dies in Berlin are filed in the vault. (Even if the person is not a Berlin resident because the Central Vermont Hospital and 2 nursing homes are located in Berlin. The Town Clerk receives records from the hospital or local nursing homes and then sends them on to the appropriate City or Town and to the State) Certified copies are available for $10.00 each.

Include in the request for a birth certificate:

                                    the child’s name,

date of birth,

an address to mail the certificate to, and

a check or money order for $10/certificate requested.

Include in the request for a death certificate:

                                    The deceased full name,

                                    date of death,

an address to mail the certificate to, and

a check or money order for $10/certificate requested.
